Test Request Form
A diagnostic specimen is any human or animal material, including excreta, secreta, blood and its components, tissue, and tissue fluids being transported for diagnostic or investigational purposes, but excluding live infected humans or animals.
A diagnostic specimen is not assigned a UN identification number unless the source patient or animal has or may have a serious human or animal disease from a risk group 4 pathogen, in which case it must be classed as Division 6.2, described as an infectious substance, and assigned to UN 2814 or UN 2900, as appropriate.
Test Request Form
A rigid or flexible portable package, other than a cylinder or portable tank, which is designed for mechanical handling.
Standards for IBC manufactured in the United States are set forth in subparts N and O of part 178 of CFR Title 49.
Gross package weight is 882 lbs or more.
Example: A corrugated bulk box of a solid material and has a gross weight of 1000 lbs.
Test Request Form
See section 173.134, 173.196 and 173.609 in CFR Title 49.
Test Request Form
A packaging that
- Consists of an outer packaging which contains articles or inner packagings.
- Is designated for mechanical handling
- Exceeds 400 kg net mass or 450 liters (118.9 gallons) capacity
- Has a volume of not more than 3m³
- Conforms to the requirements for the construction, testing and marking of large packagings as specified in the UN Recommendations, Chapter 6.6 (Orange Book)
Example: A corrugated bulk box containing 100 metal F-style cans of a hazardous solid.
Test Request Form
A waste or reusable material known to contain or suspected of containing an infectious substance in Risk Group 2 or 3 and generated in the diagnosis, treatment, or immunization of human beings or animals, research on the diagnosis, treatment or immunization of human beings or animals; or the production or testing of biological products. Regulated Medical Waste containing an infectious substance in Risk Group 4 must be classed as Division 6.2, described as an infectious substance, and assigned to UN 2814 or UN 2900, as appropriate.
See section 173.134 in CFR Title 49 for further information
